Who is a candidate for Gynecomastia - Male Breast Reduction?
In men experiencing considerable weight loss, excess skin in the chest and other areas of the body can be the result of the decrease of overall mass. For those who feel self conscious about their chest’s appearance, breast reduction surgery can help. Since the procedure removes fat and/or glandular tissue from the breasts, the result is a chest that is flatter, firmer and better contoured. In extreme cases excess skin is removed to achieve this outcome. Age is not necessarily a consideration; however the best candidates have firm, elastic skin that will reshape to the body’s new contours. In the case of massive weight loss patients, the skin may be compromised, but the results will still offer an improvement in appearance.

Surgery is not recommended for overweight men—the problem may correct itself through diet and exercise. Individuals who drink alcoholic beverages in excess, or regularly smoke marijuana are not good candidates, nor are those who take anabolic steroids. These drugs can contribute to gynecomastia. Smokers are never good candidates for any surgery since smoking decreases circulation which interferes with proper healing.

How is Male Breast Reduction Surgery done?
Surgery for gynecomastia is most often performed as an outpatient procedure unless it is combined with more extensive body procedures. It is usually done under general anesthesia, although local anesthesia plus sedation is sometimes an option. It takes about an hour and a half to two and a half hours to complete providing there are no extenuating circumstances or multiple procedures involved.

The technique to treat the male breast for MWL patients is very similar to the female reduction techniques. Typically, there is excess skin and this is the most common reason MWL patients seek correction. Various incisions can be utilized and in some cases, the nipple is removed and placed as a graft. This is done because there is less concern for attempting to maintain sensation in the male nipple.

Through these incisions the breast gland and fatty tissue can be removed. Liposuction can be used as an adjunct to finalize the chest contours. If excessive fatty tissue is the primary cause, you will likely have liposuction to remove the fat. Contouring of the chest is best done with ultrasonic liposuction. A slim tube called a cannula is attached to handle which emits ultrasonic energy. This device ”melts” the layers of fat subsequently removed using the traditional vacuum technique. The incision used for liposuction is either under the arm or along the perimeter of the areola, but is nearly imperceptible.

Recuperation after Male Breast Reduction Surgery
At the surgery’s conclusion, a drain may be inserted through a separate incision to draw off fluids. Surgical glue will cover the incisions. For patients who receive only liposuction, these are covered with a small non-adherent dressing. A compression vest may be placed to keep the skin firmly in place.

Most patients have some discomfort, but this is easily controlled with pain medication. Swelling and bruising are common. It may be a full six months to one year before you see the final results. Most patients can return to work within 1 week post op providing there is no strenuous physical activity involved in their job. Heavy exercise is discouraged for at least 3 weeks. Any sport or job that could potentially cause a blow to the chest must be avoided for 4 weeks.

Dr. Peters provides for his patient’s comfort and safety by using Physicians Surgical Specialty Hospital in Houma, LA. All procedures are done in their surgery center and recovery and aftercare is on site. The hospital staff is familiar with all the procedures Dr. Peters provides and you can expect excellent care at a reasonable price.

Your surgical fee will include:

  • Pre operative lab work
  • EKG
  • Use of the operating room and supplies
  • Intravenous antibiotics

What is involved in the initial exam?
Your initial consultation is very important. You will be asked to provide your medical history. While your candidacy for breast reduction may seem obvious and wholly related to your weight loss, underlying conditions such as liver disease, use of estrogen medications or steroids have been linked to gynecomastia, and it is prudent to rule out these potential causes. Should a medical problem be detected, you will be referred to an appropriate specialist.

Depending upon the degree of fat and glandular tissue involved, we will discuss the best surgical approach for you and answer any of your questions.

What kind of scars can I expect?
You can expect a four to five millimeter scar at the bottom of the areola when the procedure involves liposuction of the fatty tissue only. If there is a significant skin excess then the scar will extend circumferentially around the areola, extend vertically to the breast fold, then horizontally along the fold. It is in the shape of a keyhole or anchor.

When can I go out in the sun?
This is a very important question. You must avoid exposing your chest to the sun for at least 1 year. Sun can permanently affect the scar’s pigmentation, causing them to turn dark. In any case, it is advisable to use a strong sunblock (SPF 45 or greater) and be cautious.

Will my chest ever return to its preoperative condition again?
Male breast reduction is permanent. However, significant weight gain, or exposure to drugs and steroids can affect the size of your “breasts.”

How do I know if I need a lift along with a reduction?
Depending upon the amount of breast tissue and redundant skin, a breast lift may be indicated. In these cases, not only fat and breast tissue is removed, but also excess skin. An exam will be needed to definitively know if a reduction and/or lift will be necessary for the desired result.

What complications should I be aware of?
Risks and uncertainty are part of every surgical procedure. Complications associated with gynecomastia are infrequent and usually minor. Having said this, risks include infection, skin injury, excessive bleeding, an adverse reaction to anesthesia and excessive fluid loss or accumulation. It can also result in noticeable scars, permanent pigment changes or slightly mismatched breasts or nipples.
